The mission of the Center for Chronic Disease Management and Education (CCDME) is to improve the health outcomes and quality of life of individuals living with chronic diseases through comprehensive, patient-centered care, education, and collaborative healthcare services.
The Center is committed to delivering culturally responsive, evidence-based disease management and education that empowers patients to actively participate in their care and make informed health decisions.
Through interdisciplinary collaboration with primary care providers and other healthcare professionals, including pharmacist-led collaborative practice services, the Center supports optimized medication management, improved disease control, and coordinated care for patients with chronic conditions.
In addition, the Center provides healthcare consulting services to healthcare organizations, community partners, and clinical practices to support the development and implementation of effective chronic disease management strategies.
